Making Referrals to LOPA
In accordance with the Centers for Medicare and Medicaid Services (CMS) Condition of Participation (for more information, visit our CMS page), hospitals must refer to LOPA all patients meeting the clinical trigger within one hour of the first documented time. We provide cards (see below) to hospital staff. Please contact your LOPA Hospital Resources Coordinator if you need any additional cards.

If the patient cardiac arrests without meeting any of the above clinical triggers, please be sure to call in the death to our Donor Referral Line (DRL), where a LOPA representative is available 24 hours a day, 7 days a week at 1-800-833-3666.
For patients meeting a clinical trigger, we will evaluate them for organ donation potential. We will evaluate all other referrals for tissue and eye donation.
You may use this Referral Form to document your call to DRL or your hospitals appropriate paperwork.
